Wellness Grant
Salem Community Center is a private, nonprofit organization committed to the community. The Wellness Grant provides financial assistance to individuals who show the need for both financial and medical support. Financial documentation may be provided via tax returns and/or a copy of SSI or other disability statements. Medical documentation must be provided by your treating physician.
Guidelines
- Applicants must reside in the Community Center service area.
- Financial and medical documentation is required for grant consideration.
- Financial assistance grants membership for the entire membership term.
- Financial assistance is not transferable.
- Applicants must make an initial financial contribution to be considered for a grant.
Application Process
- Complete the application in full for individual membership consideration. Missing information will delay processing.
- Information provided on the application will be used for the purpose of grant review.
- Return the completed application to the SCC Front Desk. For questions about completing the application, contact the Membership department.
- Applications are reviewed each session and should be submitted by the 10th of the month prior to the desired session start. Applications received after the 10th will be held for the following session.
